Health Care for CA's in Brazil

Wikipedia states that "primary health care remains the responsibility of the federal government, elements of which (such as the operation of hospitals) are overseen by individual states. Public health care is provided to all Brazilian permanent residents and foreigners in Brazilian territory, and is free at the point of need (being paid for from general taxation)."
My questions are as follows:
1 - Do CA's participate in Brazil's health care system, which appears to be "free at the point of need"? That is, do we go to whatever doctor we choose in our area for prescriptions or say, the flu or a broken arm? Or do CA's have access to specific health care providers in Brazil who have been contracted specifically to serve U.S. diplomats?
2 - How is our health insurance used while in Brazil? If we participate in Brazil's "free" universal health care system, do we even need to purchase insurance while we're down there?

Good question...I am also curious to find out the answer.
Though from my experience hearing from those who do use the universal health care system over there, the efficiency leaves much to be desired - especially for those of us who are used to the treatment we get over here.
In short- if given the option I would gladly accept a health plan!

I have not lived in Brazil, so do not know the specific answer to your first questions. In the countries where I have lived with universal health insurance getting health care was a mixed bag - in some no problem, in others there were fees for foreigners. Whether as a diplomat you would be considered "resident" is a question. You, after all, are not paying any Brazilian taxes.
As to the need for your own health insurance plan, yes you do need to have your own stateside policy. You need this to cover any health expenditures in the States or on leave in another country. Regardless of how good Brazilian health care may be, you may find yourself being evacuated to the States for medical care and you will definitely need health insurance for this.

The Consular Adjudicators assigned to Brazil are covered by the State Department's medical program. In practice, this means that their primary health care provider would be the Embassy or Consulate Medical Units or providers. If treatment beyond the capability of a medical unit is required, it would be coordinated by those same units. The Embassy/Consulates generally use private hospitals and doctors but there could be cicumstances where you receivd treatment at a public facility. You would use your private medical insurance to cover any charges in Brazil not covered by the Embassy and for care in the U.S. if you needed treatment back here.

The public hospitals have the same doctors - excellent - but very poor/bad facilities. Only the poor use them. You would definitely want access to private health care which I'm sure is provided for. Insurance covers most everything (MRI, e.g.) without co-pay, though prescription drugs are not covered. All from my experience, of course.
