My insurance covered my bloodwork but not my physical. I had to pay for my physical out of pocket. I got receipts and an invoice for the physical, but that is different from the insurance carrier EOB that State requires. Can I still get reimbursed for my physical?

Thank you.

Please follow the instructions provided in the Candidate Guide to file for reimbursement and contact information for that office.

The guide provides an e-mail address for reimbursement of medical exams done overseas. Unfortunately, my e-mail was returned with the note that the address could not be found. Was this just a one-time computer glitch or has the address changed?

The health officer in the country where I reside and had the tests done provided me with a different address. Have not heard anything from them yet, but it was only a couple of days ago.

Thanks in advance.

From the Candidate Guide you received after passing the OA:

Where to Send your Medical Claims (Domestic Claims)
Please scan and email your information to medclaims@state.gov or fax your information to (202) 663-3858. If you have billing questions, please call our MED Finance Desk at (202) 663-1886.

Payment of Medical Expenses (Overseas Exams)
If you live overseas, you may check the website of the nearest U.S. Embassy or Consulate to find English-speaking doctors. Furthermore, if your medical exams are conducted overseas, you will have to email MEDOseasPhyExams@state.gov for guidance on the payment of your medical expenses.

A couple of days, especially during a three day weekend, is certainly not enough time to hear from MED.

I think the guy's point is that this email address (MEDOseasPhyExams@state.gov) doesn't work. I've run into the same problem. Anyone know what the real address is?

Payment of Medical Expenses (Domestic Exams)
If you choose to have your exam done by your own physician, the State Department serves as the secondary payer for all medical billings, covering the eligible amount not paid by your primary insurer. When submitting a medical claim for payment or reimbursement, you must include the following:
1. All itemized bills;
2. Explanation of benefits insurance statements;
3. Authorization form (DS-3069);
4. The tax identification number of the provider of service or facility must appear on all bills.

I'm having a hard time locating the DS-3069 form on the State website. Can you provide instructions to find it or a link?
